House Rules

• A parent or guardian is responsible for any minor under the age of 18. Minors under the age of 16 must be accompanied by an adult, who assumes full responsibility for the minor’s safety and behavior. No babies in the simulator area.

• All golf swings, including practice swings, must be taken from within the bay. Do not swing a club anywhere else in the facility, and do not step backwards prior to taking a practice swing. Maintain a safe distance from any golfer swinging a club.

• Only clean PG&S supplied golf balls and tees may be used. All other golf balls and tees are prohibited.

• No food or beverage may be brought onto the golf turf in the bay.

• Safeguard all personal property. Performance Golf & Sports is not responsible for the loss or theft of any personal property.

• Patrons are responsible for any damage they cause to the property of the tenant or the landlord of this space, including the screen, beyond ordinary wear and tear.

• No smoking or vaping in the facility.

• Please bring your own clubs. Otherwise, you can rent a set for $10/per session.

BYOB & BYOF (Beverage & Food)

• Please drink responsibly and play golf safely.

• BYOB in reasonable quantities only. Underage drinking and drinking beyond the legal limit of blood alcohol content are prohibited.

• For larger groups in the social area catering may be arranged by the group renting or by Performance Golf & Sports on behalf of the group. Special events are quoted separately from the simulators.

Assumed Waiver

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS VENUE.

• I acknowledge that Performance Golf & Sports LLC has not and will not issue on premise alcohol. Therefore all alcohol at the venue is BYOB.

• I acknowledge that I will not serve alcohol to anyone who is a minor, who is prior to the event under the influence of alcohol, who is disorderly, who is addicted to the excessive use of alcohol, or who will consume in any vehicle on the premise.

• I acknowledge that consuming alcohol can and will impair cognition, judgment, vision, motor skills, the ability to speak and will enhance aggression, agitation, and promote compulsive behavior.

• I acknowledge that Performance Golf & Sports LLC is not held liable to any property damage, charges, injuries, or deaths that may occur due to the consumption of alcohol while in their facility.

Therefore, in consideration of my application and permitting myself and party to participate in this activity, I hereby take action for myself, my group, and assigns as follows:

• (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity,

THE FOLLOWING ENTITIES OR PERSONS: The Performance Golf & Sports LLC and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers.

• (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation at this facility, whether caused by the negligence of release or otherwise.
